- Craig Sheffer (Keith Alan Scott) is featured as a "special guest star" in this episode.
- Mark Schwann, creator of OTH, appeared in this episode as Max from the Music Store.
- This is similar to the first time Lucas nearly dies in episode 1.14 "I Shall Believe." Karen pleads with a comatose Lucas, "Open your eyes." This is a recurring line beginning in the school shooting episode 3.16 "With Tired Eyes, Tired Minds, Tired Souls, We Slept." It eventually leads to Lucas discovering Keith's real killer.
- "Open your eyes" is what Jimmy Edwards said into four of the cellphones he'd confiscated from the students he held hostage at the tutor center.
- In this episode the gun lays between Jimmy and Keith. In the original school shooting episode the gun lay in front of Jimmy (who had one arm above his head instead of beside his body like in this episode) this way Dan could pick it up when Keith checked Jimmy's heartbeat.
- Goof: When Lucas has a cardiac arrest at the end of the episode, the paramedic tries to give him CPR, but instead of massaging his heart, his hands are above Lucas's stomach.
- Lucas' tattoo from season 1 he got with brooke is supposed to mean "fun", but it actually means "have."
- When the camera fades from Peyton and Lucas to Nathan and Haley in the hospital bed, you can see Haley licking her lips and opening then closing her mouth even though she's meant to be unconscious.
- Goof: When Nathan is at the bridge wishing he had died, his knuckles are fine, even though in just a few hours earlier they were bleeding after he was punching Daunte.
- The plot of the episode is an intentional reference to the movie It's A Wonderful Life. Keith, now a ghost (in a way), plays the One Tree Hill version of Clarence, and Lucas is supposed to be George.
It's A Wonderful Life is on the tv's in Lucas and Haley's hospital rooms.
